Please be sure to download your intake forms and bring them in with you. A $30 fee will be charged if you want to cancel this after it is processed. If you need to reschedule your appointment time, please give us 48 hours notice or you will be charge the full fee. Thank you. On checkout, please indicate in the comments box the time, day and location of the appointment you are requesting.
